Notes

Stevens left for Vietnam with 1 Australian Reinforcement Unit on 10 September 1969 and joined the 5th Battalion, The Royal Australian Regiment on 8 October 1969. He died from an accidentally received gunshot wound to the head at 7.30pm on 23 January 1970 in Phuoc Tuy Province, South Vietnam and is buried in grave 214, ex-services section of Woden Cemetery.
